• Health information – all clients must complete a consultation form detailing past and current health issues, medical conditions, current medications and general lifestyle choices. This allows me to customise a safe and suitable treatment for you. It is also to comply with the professional code of conduct and ethics set out by my professional body and for insurance purposes.

• Treatment records – these are my notes detailing what I have found in my assessments, what treatment I have given you and how you felt immediately after that treatment.
